NEW PATIENTS:
If you are a new patient and would like to establish care with Coastal Women's Healthcare, please call our office at 207-885-8400 to speak with a New Patient Representative. Once you have established a patient account with our practice, please fill out our medical records release form by following the link below. We will send your signed release to your previous doctor's office and obtain your medical records for you.
Before your first appointment, please take a moment to fill out your new patient paperwork by following the link below. Please be sure to bring your insurance card and co-payment with you at the time of your appointment.

You will receive your patient paperwork via text and/or email once you have been scheduled for an appointment. Your first notification will be sent 5 days prior to your appointment. If you have not received your packet please contact our office for a new link.

If you need your records sent to a facility outside of Coastal Women's Healthcare, please complete the release form below and fax it to us at 207-885-8499. We will complete your release as soon as possible.
