New Patients & Forms

New Patients:

If you are a new patient and would like to establish care with Coastal Women’s Healthcare, please call our office at 207-885-8400 to speak with a New Patient Representative. Once you have established a patient account with our practice, please fill out our medical records release form by following the link below. We will send your signed release to your previous doctor’s office and obtain your medical records for you. You will hear from a member of our Patient Services Team to schedule your appointment as soon as possible.

Before your first appointment, please be sure to create a secure patient portal account. Here, you can fill out your patient forms electronically prior to your appointment. If you do not have a chance to do this before your appointment, please arrive 15 minutes ahead of time to fill out our paper form. You can also fill out the new patient packet by printing it and bringing it in with you at the time of your appointment.

Established Patients:

All patient forms can be found on your secure patient portal account. If you do not have an account set up yet, please take a moment to do so prior to your next appointment. If you do not wish to create a patient portal account please plan to arrive at least 10 minutes early to your appointment to fill out necessary paperwork. If you have an account but are having trouble accessing it, please call 207-885-8400 and speak to a member of our Patient Services Team, they will be happy to assist you.

If you need records sent to a facility from Coastal Women’s Healthcare, please complete the release form below and fax it to us at 207-885-8499. We will complete your release as soon as possible.
